This survey will provide us with basic information about your hair and it will require you to tell us about specific products that you use. Thus, it may be ideal for you to be in close proximity to them for you to reference. The more detailed you are, the better we will be able to help you.
Please read these instructions before beginning the survey.
- Allow approximately 10-15 minutes to complete the survey. All fields are required.
- If you choose to leave the survey and resume it later, the “Next” link on a page must be clicked to save that page of answers. When you resume, the survey will begin where you left off.
- Do not delete the email invitation until the survey has been completed as this will allow you to reconnect to the survey at your convenience.
- Click on the “Done” link at the end of the survey when you are finished to save all your answers and submit the survey.
